Company Details
This must be the exact address that is stated in the rental agreement of the company which include street, number, postcode, and city.
Briefly describe the business activities (e.g. "trading in software products and provision of IT consultancy services"). This will be included in the articles of association.
Leave blank if flexible. The notary will confirm availability.

Persons Involved
Please provide the following details for each director and shareholder separately. If there are multiple persons, list each one clearly (e.g. "Person 1: … | Person 2: …").
Include all first names and surname(s) exactly as they appear on the identity document.
Format: DD-MM-YYYY. List one per person.
City and country for each person.
Private residential address for each person — not the company address.

Shareholding & Authorization
Any person holding 25% or more of the shares must be registered in the UBO (Ultimate Beneficial Owner) register. This information is required by law.
List each shareholder with their percentage. Must total 100%.

Compliance & Source of Funds
As a regulated service provider, Rena Consultancy is required under the Dutch WWFT (anti-money laundering law) to collect and verify information on the origin of funds and politically exposed persons.
e.g. personal savings, business income, loan from a family member, inheritance. Please be specific.
A PEP is a person who holds or has held a prominent public function (e.g. politician, senior government official, judge, senior military officer).

Identity Documents
The notary is legally required to verify the identity of all directors and shareholders before incorporation. Please upload a clear copy of a valid passport or national ID card for each person.
